I have received a question by a reader. She wanted to know about unsweetened rice milk. She was aware of unsweetened soy milk but she was looking for the rice milk instead.

Well, she might try the same way we make nut milks. To make nut milks, you just soak the nuts over night, toss the nuts and the milk in the blender and then you have nut milk. Perhaps rice milk was made in the same way.

Or she could just get Rice Dream.

Unfortunately Rice Dream is sweetened, so it’s bad for those with candida problems. It may not say anything on the box, but there is sweetener in it. They usually use brown rice syrup, or malt syrup or organic cane juice or maltodextrine, etc. If it just lists brown rice, then consider it using brown rice syrup. If it tastes sweet, it’s been sweetened with something. On the nutritional label it will list the amount of sugar in it in the carbohydrates section. If it has more than a couple grams of sugar, it is sweetened with something.

There is a web site with tons of recipes for making your own rice and there are some unsweetened rice milk recipes on it. However, white rice can be a problem for people with candida as well.

Sally Fallon, the author of Nourishing Traditions cookbook just held a lecture and she felt that it was difficult for bodies to process the soy. So it might be a better idea to switch to rice milk instead. But knowing that unsweetened rice milk is difficult to find, you might try diluted coconut milk. Coconut milk is pricey, but a can will last several days if watered down, and as well as being anti-fungal, it tastes a lot nicer than soy.

Want to know more about unsweetened things for the diet or anything? Join me at cure candida project website and get the full report about it.

Each EOB is specific to the provider (physician, lab) and the service they rendered. The format varies widely between insurance companies, but at a minimum an EOB should indicate the: provider, service date, actual billed amount, network discount, allowed amount, insurance portion, patient responsibility and deductible amount. I’ll discuss each of these terms. It is important to remember that an EOB is NOT a bill. Usually, almost all EOBs state “THIS IS NOT A BILL” in big bold letters, to minimize confusion.

Every EOB you contains information about services you received from that specific provider during a particular visit. A provider is anyone who supplies healthcare and/or other medically related services and products. A provider may be a physician, dentist, clinic, hospital, pharmacy, lab, physical therapists or other health care professional. Looking at your EOB, you should see the name of the provider or facility that filed the claim for reimbursement listed. A service is a procedure or product rendered by a provider to a patient. A service could range from a basic physician visit to radiology services to surgical care or medical equipment. The date on which you received services from a provider is called the service date.

Billed Amount

This represents the cost of the services you received and the amount the provider sent to your insurance company. It should be the largest figure on your EOB.

Network Discount

This is the amount by which a providers bill is adjusted as a result of a negotiated rate agreed upon between the provider and the insurer, and is not always listed on an EOB. This only applies to in-network providers.

Allowed Amount/Covered Amount

The allowed amount is the amount of payment a provider has agreed to accept for the service, treatment or product under the terms of a negotiated contract with an insurance company. This applies only to in-network providers. The allowed amount may also be the maximum amount the insurance company will allow for a specific service.

Insurance Amount/Paid

As you may have already guessed, this is the amount that your insurance company pays on a claim.

Deductible Amount

Your deductible indicates the portion of expenses that count toward your plan deductible and you are responsible to pay.

Patient Responsibility

This is the portion of the provider charges to be paid by the patient after the network discount, allowed amount, insurance portion and deductible amount have been taken into account. Basically, this is what you are expected to pay out-of-pocket for the services your received.

The Math

The key to understanding comes down to identifying four numbers: the provider charges (amount billed from doctor, hospital, etc.), the discount (which is based on the negotiated rate of your individual plan coverage), what insurance paid and patient responsibility.

The amount the provider sent to your insurance company as their “charge” or “billed amount” should be the largest figure on the EOB. Next, find and subtract the “discount” to arrive at the allowed amount, which is the amount your insurance company and your provider agree is the fair amount to be paid. If you do not have a discount amount displayed, simply locate the allowed amount on your EOB.

Now look at the amount your insurance paid. It can be anywhere from $0 to the full allowed amount. Subtract what the insurer paid from the allowed amount. What’s left is the patient responsibility. To double check the math, add together the amount the insurance paid and the patient responsibility, it should equal the allowed amount, sometimes referred to as the negotiated rate.

Remember, the patient responsibility may NOT be the balance owed to your provider, depending on if the insurance company reflects your payments already to the provider (such as co-pays, pre-payments and any other payments you have submitted). If payments you have previously made are NOT reflected on the EOB, you will need to subtract your payments from the patient responsibility amount to figure out the balance owed to the provider.

EXAMPLE:

Billed Office visit $155.00

Allowed Amount $ 93.03 (Network Discount $61.97)

Plan Pays $83.73 (Insurance Paid)

Patient Responsibility $ 9.30

These days less and less surgery is being performed in hospitals and more and more surgery is being performed in medical offices outside of the normal hospital system. Think of these places as little centers that are set up by doctors themselves to perform minor surgeries much cheaper than at the major hospitals.

These things range from laser eye surgery centers to wart removal centers and other things like that.

Before you use one of these places for which ever type of surgery you require, here are several questions you should ask the doctors involved before you have the surgery. In some cases these questions could very well save your life.

The first question to ask your doctor is whether or not they have hospital privileges to perform the procedure that they will be performing on you in the office center. Hospitals only grant hospital privilege to doctors who have undergone a lot of scrutiny. Generally speaking you only want to use a doctor who does have hospital privileges. Failure to have these privileges may be an important red flag for you to keep in mind.

The next question to ask is how many similar procedures has the doctor performed. Surgery is like anything else in life, the more often you do it the better at it you get. You should only use surgeons who have done the operation many many times in the past with successful results.

The next question to ask is whether or not the office is prepared for emergencies. At the very minimum the office should have a crash cart that is equipped with a defibrillator. Life support equipment is also a plus. When it comes to this sort of thing, more is obviously better.

The next question to ask is whether or not the doctor has an ongoing relationship with a specific ambulance company. This is important because especially in a large city, ambulance drivers have to be familiar with the location of the office in order to find it quickly. They also need to know the quickest routes from the office to the hospital and without an ongoing relationship they might not know it. Seconds count after all.

Finally you should ask whether or not the office has been accredited. Depending on the services they’re offering different agencies may be involved in certification and accreditation. Don’t be shy about asking to see these certificates of accreditation. Most doctors will have these hanging on the wall in the reception room anyway but it never hurts to ask and no doctor should feel bad about you asking.

So there you have several questions you should ask your doctor before having surgery in an off hospital facility of any kind. When it comes to this sort of thing, the more questions you ask the better because this is your health we are talking about, and nothing is more important than that.

Whilst the health agenda in the USA has captured most of the headlines, the same issues are confronting other countries too. The central issue remains how to pay for the services, which are expected to be needed, by an aging population with a shrinking taxpayer base.

In Australia a tax review has just been completed and the third inter generational report is about to be handed down. This looks at projections for the next generation.

One of the snippets from the tax review caught my eye. It claimed that the cost per head of pharmaceuticals in Australia had trebled in twenty years from around $200 per head to over $600 whilst in the preceding twenty years it had remained stable. This was put down to the development of “blockbuster” drugs.

The cost of drugs in Australia is much less than in the USA due to a system called the Pharmaceutical Benefits Scheme, which in simplest terms makes the government the “sole” buyer of drugs that are listed on the scheme. So where is most of the money going?

By far the biggest spend is on cholesterol lowering drugs (statins) which accounted for AUD $1billion in 2008/09. Other biggies were reflux drugs and blood thinners at around AUD $200 million each and a common antidepressant at AUD $100 million.

The next question is what are we getting for our money? Life expectancies have continued to gradually increase over the last century by about one quarter of a year per year. This rate has not accelerated over the last twenty years. Rates of heart disease and reflux have not plummeted.

Given the amount expended on pharmaceuticals you would think that questions about benefits would be asked. By this I do not mean clinical trials, which shows that the drug “works”, I mean questions about benefits to society in terms of longevity or better levels of health. There seems to be silence on that front.

In the whole “health care” reform agenda there are always arguments about how we are going to afford the cost of disease. There are never any questions asked about ways to reduce spending on disease by people being…healthier. When one looks at the “conditions” treated by the most expensive drugs they are mainly lifestyle conditions and the cleverly coined term” risk factors”.

In other words we are treating with expensive tablets that which we have brought about through our habits be they dietary, exercise or other. Logically then, if it was not a pill that caused the problem, why is a pill the answer?

What is needed is a change in the way we look at health care. Our current systems are disease systems. The cost comes from treating disease. There is little or no support for or investment in people being healthy. A genuine health system would be looking to keep people healthy rather than pick up the pieces after they have become sick.

As individuals we need to be responsible too. Around three quarters of spending on disease is for diseases that come about not through random action but by our daily actions. We need to be responsible for our own health. Here are eight simple tips

1) Each day do some slow deep breathing
2) Make water your main beverage
3) Eat food that till recently was moving around or growing somewhere
4) Be active each day
5) Get adequate, eight hours for most people, sleep
6) Manage your stress
7) Have good relationships
Have fun and purpose in your life

This is all easy and inexpensive. All that is required is for you to do it.

While Medicare cuts benefits for patients that need them, they are throwing money out the window renting oxygen equipment. Medicare has paid over $200 per month for over 10 years for some patients oxygen benefits. Now Medicare put a 36 month cap on oxygen benefits because they are running out of money. Whose fault is that? They will only pay for a patients benefits for 36 months rather than just buy a brand new oxygen concentrator for the patient that needs it. A brand new Respironics Everflo Q or Invacare Perfecto 2 home oxygen concentrator could be found brand new on the internet for about $600. Why don’t they just buy the oxygen concentrators outright instead of paying $200 per month and wasting all that money. Patients are losing out because Medicare does not know how to budget there money. Its common sense to the average person, pay a one time fee of $600 or pay $200 times the cap of 36 months, which comes to $7,200.

Would it not make sense to save money and just buy the oxygen concentrator outright for the patient?
Today, Medicare will make a one time payment and buy out an electric wheel chair for $4000. That is why you can turn on your t.v. and see a commercial offering you a free electric wheelchair. Why not just buy an oxygen concentrator outright and just replace the unit if something happens to it? Our population is getting older and if our government keeps throwing the money away there is not going to be any left for the up and coming oxygen therapy patients.

I realize that these home care companies have to make a living but there has to be a solution. Everyone is being hurt. The home care companies are only being compensated for 36 months and asked to continue service for 2 more years without being paid. What type of service is a patient going to get if the home care company is no longer being paid for servicing that patient? Not much. Lets hope the right person reads this article and realizes it is better to buy a new home oxygen concentrators for $600 rather that buy the same exact unit for $7200.

Renting and selling portable oxygen concentrators throughout the U.S.A.

Hand surgery is a complex procedure which requires the expertise of a surgeon especially trained in treating the upper extremities. Hand surgeons are trained even more intensely than a regular orthopedic surgeon. An orthopedic surgeon specializes in treating the bones, ligaments, tendons and muscles of the skeletal system.

A hand specialist has the training of a traditional orthopedic surgeon along with additional schooling in the hand, wrist and shoulder. Most surgeries are successful and offer the patient more flexible movement in their hand, wrist and upper extremities and the alleviation of pain. Some complications do occur, however, and here are some of the more common ones:

- Delayed healing due to inadequate blood circulation. Steps are taken to minimize this including excessive debridement, which is the removal of dead tissue, and careful attention to the vascular reattachment.

- Edema or swelling can complicate healing, as well. Elevation and moving the area according to the physician’s instructions will help keep edema to a minimum.

- Infection can occur at the wound site but the chance of this happening is minimized by preventive antibiotics given before and after the surgery. Gentle exercise, as instructed, will also keep an infection from taking hold.

- Improper positioning of splints and dressings which are too tightly wound can cause problems with the post-op healing by excessive pressure or inadequate support.

- An adverse reaction to anesthesia that is used during surgery is a risk in any surgical procedure.

- Nerve damage by needle puncture can occur. If this happens, there would be resulting tingling and numbness in the area.

- The use of tourniquets during surgery can result in damage to the radial nerve including paralysis. This is extremely rare.

- Toxic shock syndrome is also a rare occurrence in surgery. This is a life threatening infection caused by bacteria.

- Excessive bleeding during the operation is another complication. The surgeon will instruct the patient to curtail certain medications and herbal supplements which are known to cause heavy bleeding.

- Phlebitis is another rare complication. Phlebitis is the medical name for blood clots in the veins.

Most hand surgeries are successful, but complications do occur. Choosing a reputable specialist and practicing self care before and after the procedure will greatly minimize their occurrence. Avoiding cigarette smoking, getting proper rest along with a healthy diet will go a long way in supporting the immune system.

The truth is, looking for a Medicare Supplement quote can be a pleasant experience, or one that you may regret for a long time. Here is why…

The hard way:

There are many, many websites that will tell you that they offer a quote for your Medicare Supplement, but when you fill out the form and give them your information, they actually sell your information to the highest bidders (usually 10 or more agents)!

You will get inundated with calls and emails, and I can assure you, it will not be a pleasant experience.

The easy way:

Find a licensed online Medicare Supplement brokerage that will compare all of your options for you, answer all of your questions and help you find the best plan for you at the lowest possible price.

Comparing Medicare Supplemental quotes is actually very easy.

In most states (except for three), Supplemental plans are all standardized. What this means is that a Plan F from Company 1 offers the same exact benefits as a Plan F from Company 2. The only real difference is the price.

For example, Mary bought a Plan F from Company 1 and the monthly premium was $189 per month. When Mary came to us, we got her the “same” Plan F from Company 2 for $119 per month.

Just by comparing companies, Mary saved $70 per month, or $840 per year!

Whether you are turning 65 or simply shopping for lower rates, you can easily compare companies and save on your Medicare Supplement as well.

The key is to find an online brokerage you can trust. A good online brokerage will help you decide without any pressure and will be able to compare all of your options where you live.

To get your Medicare Supplement Quote go now to: MedicareNational.com.

The Cost of Our Food Should be Measured in the Total food, Health Care, Energy, and Environmental Costs of What We Choose to Eat. Looked at this way, it is insane how much we are paying and future generations will be paying for our unhealthy diet pleasures. Meanwhile, half the world is starving and the other half remain overfed.

It is remarkable that the American public, as tight as money is, is so fearful about some unexpected health crisis popping up, that we are willing to pay hundreds and even thousands of dollars per family each month for health insurance, which may get cancelled at the last moment anyway. This is a perfect example of how we have been conditioned to fear things we don’t understand.

Rather than taking the time to educate ourselves on how to eat to stay both thin and well, we allow ourselves to be manipulated by our fear of some nameless health catastrophe. And then, overcharged for high-tech solutions that treat only our symptoms. (To be fair to doctors, they can’t control what we eat.)

And yet, for all the “protection money” we spend, we are unwilling in most cases to do the things we are told by public health agencies that will prevent many of the most feared diseases. We pay exorbitant charges for health care insurance rather than correct our personal risk factors.

This being January, the month when most Americans decide to get serious about dieting for a week or two, it may be appropriate to look at how insane our dietary habits are.

The diseases that trouble us the most are caused by the junk we eat. Most of them can be prevented by correcting our diet–once and for all–and learning to ignore temptation. We should recognize slick advertisers of unhealthy foods as the Devil’s own representatives–and avoid their recommendations like the plague.

What we do instead, with the encouragement of our medical providers, is spend $70-100+ per month for a statin drug to allow us to go on eating bacon and eggs, meats, and ice cream, without worrying about our cholesterol levels. Would it not be simpler and healthier–and cheaper!–to stop eating extra cholesterol? But no, life in modern America wouldn’t be worth living without being free to graze on all the delectable goodies that food chemists concoct for us each day–or so we appear to think.

So, because 68% of Americans are overweight, which should be an easy problem to remedy with a little self-discipline, we are prone to become diabetic, suffer from cardiovascular disease, and several cancers. This we know. What are we going to do about it this year? Another bout with the Atkins diet? Or will it be some new diet that is guaranteed to be a marketing success and a weight loss failure?

Weight loss is generally viewed as a simple math problem. Usually, we’re given the old choice: We’ve either got to exercise a lot more or eat a lot less, but most dieters want to do neither one.

There is, however another option that most of us ignore with a passion. And that is changing WHAT we eat rather than limiting how much we eat. Specifically, we’re talking about cutting out meats, eggs, and milk products.

That is the key that few of us want to use to unlock the weight loss riddle. More than being a problem of eating too much, weight gain is caused by eating the wrong kind of foods. Anthropologists point out that our digestive systems are not set up to be carnivorous. But since McDonald’s and Pizza Huts even provide the food service in many public schools now–and have for some time–most of us find it hard to imagine life worth living without fast foods, fried foods, and junk snack foods.

We have been trained to eat the wrong foods and to expect someone else to solve the medical problems this creates. (And now, it looks like we will all be forced to buy health care insurance, whether we want it or not.)

Health care, as we currently see it practiced, is not the answer, though.

Until we are each able to see the part we play in allowing ourselves to be manipulated by advertisers to eat unnatural, unhealthy foods, we will not be ready to take the first step to permanent weight loss. Short term changes in what we eat, diet pills, and all the new crop of diet books will not produce the desired results until we learn to see how we have been duped by food processors and our government-subsidized industrial system of farming.

We need real food, grown in healthy soils, not colorful boxes of factory-farmed “foods” with minimal nutrients and maximal flavor additives. And, we need to stop eating animals and their by-products. The alternative is to continue to pay outrageous prices for continuing to eat as we do.

One option to the unrealistic expectation of converting the nation to organic farming methods overnight is to encourage everyone to supplement their diet with high-quality vitamin and mineral supplements. It makes sense that much of our overeating is an unconscious urge to get the vital nutrients that should be in our food but aren’t, due to depleted soils.

If it seems extreme to suggest we stop eating meat, other animal products(milk, etc.), and processed foods, don’t you think open-heart surgery, knee and hip replacements, kidney dialysis on a regular basis, chemotherapy, and Alzheimer’s Disease are extreme, too? Countries where the populations live on simple, naturally grown foods, with little meat in their diets rarely get the diseases we do. We should learn from them.

If, as 45-year cancer researcher T. Colin Campbell tells us, our diet is responsible for 97% of the diseases we get, then it is up to us to stop offering ourselves up as prey for the health care industry. Change your diet and you can change your medical fate.

The irony of this whole milieu of sickness, obesity, and high health care costs is that we are doing it to ourselves by what we choose to eat. As soon as we realize that fact, we can start doing something to reduce our weight and our health care expenses.

As a happy side-effect, changing our unhealthy diet will take a huge burden off our environment and be a good example for the rest of the world.

There has been a convergence of several factors all at one time that has created a sea-change in America that will make some of us insanely rich over the next 3,5 & 7 years while others will be driven and pushed further and deeper into states of depression, joblessness & even poverty!

The ever popular baby-boomers who happen to not only be the single largest group of people in America, they also represent over $5 trillion dollars of the $10 trillion dollar economy plus they literally drive the entire economy. So because this huge demographics of adults are now rapidly aging they have fueled a health & wellness industry that has nearly topped $1 trillion dollars in sales!

It is these baby boomers who have fueled the popularity of all things related to looking better, living longer and feeling great while doing so! They also have a vain love for all things that reverse or slow the effects and impact of aging which is why the plastic surgery industry has rocketed into the atmosphere like a space shuttle. People are opting for breast reductions, augmentations, tummy tucks, gastric bypass surgeries, nose jobs, lip injection and a host of other modern day spa & skin treatments and a host of other high priced treatments & procedures!

In fact, by the year 2050 the total medical expenses will exceed the overall Gross Domestic Product of the Unisted States so as you can see health & wellness and medicine are literally bigger than America itself! So provided all of that is true (and it is) then what is the average American capitalist like you and I to do!

Seize the opportunity of a lifetime while the opportunity still has life! You see the above mentioned events all create a tremendous opportunity for the savvy businessman or woman whose antennas are tuned into recognizing opportunity!

For the last 15 months, people all over the US and 6 countries have perfected a genius delivery system and product that is unique to the civilized world and habitually consumed without regard to price or taste! Take a product that is consumed habitually without regard to price and taste and then add to that equation health and you literally have your hands on the next BILLION DOLLAR opportunity, plain and simple!

In an emergency situation, time is of the essence. Your medical care providers will need as much information as possible about your medical condition. Having a medical organizer can be a real lifesaver.

When you are not able to provide your health history in an emergency, a medical organizer can speak for you. Today, with the use of the internet, medical professionals can have instant access to your medical records.

Your Online Personal Health Record

In an ideal world, emergency room staff would like to have as much information about your health history as possible. In the past, this was virtually impossible unless you carried all of your medical records in your pocket.

Today, however, it is possible to provide emergency room medical personnel with all of your health information through the use of an online personal health record. Online access allows physicians to know your medical conditions, allergies, medication, family contact information and health care directives.

Medical Privacy Concerns

Protecting the privacy of our health information is a major issue in health care. This becomes a challenge when you rely on instant access of information via the internet.

The use of new encrypted technology now provides increased protection. A secure web-based system allows healthcare professionals access to your personal health record via your name and password.

Minimize Delays in Treatment

In a true emergency, seconds count. The use of a web-based electronic personal health record will provide emergency room staff with instant access to your vital medical history.

Good communication is an essential element in successful outcomes. If you are unable to speak for yourself, having a personal health record speak for you is the next best thing.
