Various healthcare sectors have published numerous safety guidelines and measures that are highly efficient. These safety guidelines help to minimize the risks of exposures to hazardous drugs. The healthcare workers who are involved in handling of hazardous drugs are more prone to its fatal affects. The growing concern for the safety of these healthcare workers forced the healthcare units to formulate the safety guidelines and designing of several safety equipments. A needle-less system is introduced for the safety of the workers that will provide them safety against the needle-stick injuries.

A vial adaptor is a safety device that is used for the transfer of fluid from a vial to a needle-less syringe. This vial adaptor has a top end, an inner cavity and a bottom end. The top end is sealed by a septum. The inner cavity is designed to receive the vial and the bottom end extends below the lower part of the vial. There is a needle within the body that is used to puncture the septum. A luer lock is present on the top end of the body. A radial wall separates the top and bottom portion of the luer lock. A valve is present between the top and the bottom portion to control the movement of fluid. The valve automatically opens when the vial adaptor is attached to the needle-less syringe.

The vial is a non-sharp plastic device that can fit into all the standard syringes available. Without the help of a needle, the syringe can be filled with medications. Hence, it is a very simple and a safe method for the transfer of drugs for reconstitution purpose. Reports have shown that healthcare workers suffer numbers of injuries from conventional needles. Therefore the introduction of needle-less system has minimized the risks of exposures to hazardous drugs to a great extent.

For more information on Exposures to hazardous drugs and the benefits of Vial Adaptor please visit the mentioned website.

There is currently a debate regarding using and maintaining electronic medical records. Much of this debate is between people being “forced” to store their records this way. For those who choose to store their records electronically as the preferred method, there are some advantages to having your medical records stored this way. Learn what some of these are to help you make the choice about whether or not to utilize this method.

First of all, electronic medical records are designed for quick and easy access. One doctor to the next can look at them and find out what you have had done to you, what tests have been run on you, and other items in your history. This is much easier than when your records are stored on paper.

Doctors will get a full picture on what your health actually is. With paper records at times things get fragmented and lost. Electronically things are kept in one place so that a more clear picture is found.

All of your new information can be easily and quickly updated so that your records are always up-to-date. It’s important to have the latest information so that you can get the best medical care possible knowing what’s going on in your body.

You can also save money on tests. If a doctor suspects a certain disease or illness, he or she might want to run a particular test on you. If this has been done in the past, you might be able to skip future tests and save your time and money during this process.

You can save time with a new doctor. Instead of putting in the information over and over, the doctor can access your electronic files getting all the information. Some people might forget things that are already on their electronic records.

These types of records are stored online but are fairly safe. Your privacy is important and the security is exceptional as it lets you have these advantages while keeping your privacy.

Each EOB is specific to the provider (physician, lab) and the service they rendered. The format varies widely between insurance companies, but at a minimum an EOB should indicate the: provider, service date, actual billed amount, network discount, allowed amount, insurance portion, patient responsibility and deductible amount. I’ll discuss each of these terms. It is important to remember that an EOB is NOT a bill. Usually, almost all EOBs state “THIS IS NOT A BILL” in big bold letters, to minimize confusion.

Every EOB you contains information about services you received from that specific provider during a particular visit. A provider is anyone who supplies healthcare and/or other medically related services and products. A provider may be a physician, dentist, clinic, hospital, pharmacy, lab, physical therapists or other health care professional. Looking at your EOB, you should see the name of the provider or facility that filed the claim for reimbursement listed. A service is a procedure or product rendered by a provider to a patient. A service could range from a basic physician visit to radiology services to surgical care or medical equipment. The date on which you received services from a provider is called the service date.

Billed Amount

This represents the cost of the services you received and the amount the provider sent to your insurance company. It should be the largest figure on your EOB.

Network Discount

This is the amount by which a providers bill is adjusted as a result of a negotiated rate agreed upon between the provider and the insurer, and is not always listed on an EOB. This only applies to in-network providers.

Allowed Amount/Covered Amount

The allowed amount is the amount of payment a provider has agreed to accept for the service, treatment or product under the terms of a negotiated contract with an insurance company. This applies only to in-network providers. The allowed amount may also be the maximum amount the insurance company will allow for a specific service.

Insurance Amount/Paid

As you may have already guessed, this is the amount that your insurance company pays on a claim.

Deductible Amount

Your deductible indicates the portion of expenses that count toward your plan deductible and you are responsible to pay.

Patient Responsibility

This is the portion of the provider charges to be paid by the patient after the network discount, allowed amount, insurance portion and deductible amount have been taken into account. Basically, this is what you are expected to pay out-of-pocket for the services your received.

The Math

The key to understanding comes down to identifying four numbers: the provider charges (amount billed from doctor, hospital, etc.), the discount (which is based on the negotiated rate of your individual plan coverage), what insurance paid and patient responsibility.

The amount the provider sent to your insurance company as their “charge” or “billed amount” should be the largest figure on the EOB. Next, find and subtract the “discount” to arrive at the allowed amount, which is the amount your insurance company and your provider agree is the fair amount to be paid. If you do not have a discount amount displayed, simply locate the allowed amount on your EOB.

Now look at the amount your insurance paid. It can be anywhere from $0 to the full allowed amount. Subtract what the insurer paid from the allowed amount. What’s left is the patient responsibility. To double check the math, add together the amount the insurance paid and the patient responsibility, it should equal the allowed amount, sometimes referred to as the negotiated rate.

Remember, the patient responsibility may NOT be the balance owed to your provider, depending on if the insurance company reflects your payments already to the provider (such as co-pays, pre-payments and any other payments you have submitted). If payments you have previously made are NOT reflected on the EOB, you will need to subtract your payments from the patient responsibility amount to figure out the balance owed to the provider.

EXAMPLE:

Billed Office visit $155.00

Allowed Amount $ 93.03 (Network Discount $61.97)

Plan Pays $83.73 (Insurance Paid)

Patient Responsibility $ 9.30

Whilst the health agenda in the USA has captured most of the headlines, the same issues are confronting other countries too. The central issue remains how to pay for the services, which are expected to be needed, by an aging population with a shrinking taxpayer base.

In Australia a tax review has just been completed and the third inter generational report is about to be handed down. This looks at projections for the next generation.

One of the snippets from the tax review caught my eye. It claimed that the cost per head of pharmaceuticals in Australia had trebled in twenty years from around $200 per head to over $600 whilst in the preceding twenty years it had remained stable. This was put down to the development of “blockbuster” drugs.

The cost of drugs in Australia is much less than in the USA due to a system called the Pharmaceutical Benefits Scheme, which in simplest terms makes the government the “sole” buyer of drugs that are listed on the scheme. So where is most of the money going?

By far the biggest spend is on cholesterol lowering drugs (statins) which accounted for AUD $1billion in 2008/09. Other biggies were reflux drugs and blood thinners at around AUD $200 million each and a common antidepressant at AUD $100 million.

The next question is what are we getting for our money? Life expectancies have continued to gradually increase over the last century by about one quarter of a year per year. This rate has not accelerated over the last twenty years. Rates of heart disease and reflux have not plummeted.

Given the amount expended on pharmaceuticals you would think that questions about benefits would be asked. By this I do not mean clinical trials, which shows that the drug “works”, I mean questions about benefits to society in terms of longevity or better levels of health. There seems to be silence on that front.

In the whole “health care” reform agenda there are always arguments about how we are going to afford the cost of disease. There are never any questions asked about ways to reduce spending on disease by people being…healthier. When one looks at the “conditions” treated by the most expensive drugs they are mainly lifestyle conditions and the cleverly coined term” risk factors”.

In other words we are treating with expensive tablets that which we have brought about through our habits be they dietary, exercise or other. Logically then, if it was not a pill that caused the problem, why is a pill the answer?

What is needed is a change in the way we look at health care. Our current systems are disease systems. The cost comes from treating disease. There is little or no support for or investment in people being healthy. A genuine health system would be looking to keep people healthy rather than pick up the pieces after they have become sick.

As individuals we need to be responsible too. Around three quarters of spending on disease is for diseases that come about not through random action but by our daily actions. We need to be responsible for our own health. Here are eight simple tips

1) Each day do some slow deep breathing
2) Make water your main beverage
3) Eat food that till recently was moving around or growing somewhere
4) Be active each day
5) Get adequate, eight hours for most people, sleep
6) Manage your stress
7) Have good relationships
Have fun and purpose in your life

This is all easy and inexpensive. All that is required is for you to do it.

While Medicare cuts benefits for patients that need them, they are throwing money out the window renting oxygen equipment. Medicare has paid over $200 per month for over 10 years for some patients oxygen benefits. Now Medicare put a 36 month cap on oxygen benefits because they are running out of money. Whose fault is that? They will only pay for a patients benefits for 36 months rather than just buy a brand new oxygen concentrator for the patient that needs it. A brand new Respironics Everflo Q or Invacare Perfecto 2 home oxygen concentrator could be found brand new on the internet for about $600. Why don’t they just buy the oxygen concentrators outright instead of paying $200 per month and wasting all that money. Patients are losing out because Medicare does not know how to budget there money. Its common sense to the average person, pay a one time fee of $600 or pay $200 times the cap of 36 months, which comes to $7,200.

Would it not make sense to save money and just buy the oxygen concentrator outright for the patient?
Today, Medicare will make a one time payment and buy out an electric wheel chair for $4000. That is why you can turn on your t.v. and see a commercial offering you a free electric wheelchair. Why not just buy an oxygen concentrator outright and just replace the unit if something happens to it? Our population is getting older and if our government keeps throwing the money away there is not going to be any left for the up and coming oxygen therapy patients.

I realize that these home care companies have to make a living but there has to be a solution. Everyone is being hurt. The home care companies are only being compensated for 36 months and asked to continue service for 2 more years without being paid. What type of service is a patient going to get if the home care company is no longer being paid for servicing that patient? Not much. Lets hope the right person reads this article and realizes it is better to buy a new home oxygen concentrators for $600 rather that buy the same exact unit for $7200.

Renting and selling portable oxygen concentrators throughout the U.S.A.

The truth is, looking for a Medicare Supplement quote can be a pleasant experience, or one that you may regret for a long time. Here is why…

The hard way:

There are many, many websites that will tell you that they offer a quote for your Medicare Supplement, but when you fill out the form and give them your information, they actually sell your information to the highest bidders (usually 10 or more agents)!

You will get inundated with calls and emails, and I can assure you, it will not be a pleasant experience.

The easy way:

Find a licensed online Medicare Supplement brokerage that will compare all of your options for you, answer all of your questions and help you find the best plan for you at the lowest possible price.

Comparing Medicare Supplemental quotes is actually very easy.

In most states (except for three), Supplemental plans are all standardized. What this means is that a Plan F from Company 1 offers the same exact benefits as a Plan F from Company 2. The only real difference is the price.

For example, Mary bought a Plan F from Company 1 and the monthly premium was $189 per month. When Mary came to us, we got her the “same” Plan F from Company 2 for $119 per month.

Just by comparing companies, Mary saved $70 per month, or $840 per year!

Whether you are turning 65 or simply shopping for lower rates, you can easily compare companies and save on your Medicare Supplement as well.

The key is to find an online brokerage you can trust. A good online brokerage will help you decide without any pressure and will be able to compare all of your options where you live.

To get your Medicare Supplement Quote go now to: MedicareNational.com.

In an emergency situation, time is of the essence. Your medical care providers will need as much information as possible about your medical condition. Having a medical organizer can be a real lifesaver.

When you are not able to provide your health history in an emergency, a medical organizer can speak for you. Today, with the use of the internet, medical professionals can have instant access to your medical records.

Your Online Personal Health Record

In an ideal world, emergency room staff would like to have as much information about your health history as possible. In the past, this was virtually impossible unless you carried all of your medical records in your pocket.

Today, however, it is possible to provide emergency room medical personnel with all of your health information through the use of an online personal health record. Online access allows physicians to know your medical conditions, allergies, medication, family contact information and health care directives.

Medical Privacy Concerns

Protecting the privacy of our health information is a major issue in health care. This becomes a challenge when you rely on instant access of information via the internet.

The use of new encrypted technology now provides increased protection. A secure web-based system allows healthcare professionals access to your personal health record via your name and password.

Minimize Delays in Treatment

In a true emergency, seconds count. The use of a web-based electronic personal health record will provide emergency room staff with instant access to your vital medical history.

Good communication is an essential element in successful outcomes. If you are unable to speak for yourself, having a personal health record speak for you is the next best thing.

Before the carrier companies started various modes of voice mails, answering service was the one that tackled all the customers. In business like health care, easy accessibility of different clinical services is must. More and more health care centers and clinics now have medical answering service for a smooth clinic management. Medical answering service answers the phones and takes messages from patients.

Medical answering machine installation cuts off the cost and is very reliable. The patients say that they never get busy tone and they never fail in delivering their messages. On the other hand, health care providers can be sure of getting all the messages and hence can remain up-to-date about their patients’ scheduling. Live answering service can build a very good rapport with patients and hence can boost the medical business.

The best part of this system is it is 24/7 medical answering system that can help patients making appointments while most live agents find it difficult to do it for your clinic. By installing such system as a part of office automation, the health care providers and doctors can have uninterrupted after-hour answering service so that they do not miss any potential customers in the form of patients. The medical practice management becomes smoother and physician answering service can get patients a full satisfaction making them permanent clients.

One can also use a medical answering system as a virtual front desk receptionist answering the calls of patients and solving the queries they have. Also, as compared to a real receptionist, answering machine is much more dependable, efficient and above all, friendlier. Medical answering machine service is capable tackling multiple calls at the same time and hence, patients will never be kept on hold. Also, dispatching emergency calls without any delay and human errors is quite possible.

There are many benefits that come with automated answering mode such as -it has no excuses! It never gets tired and does not require sleep. This assures attending every potential customer, the patient round the clock giving appointments or solving the queries. Surveys and feedbacks of patients say that such automated answer machine is much better and efficient than a live agent medical assistant. Also, such system is maintenance free and does not require much attention once the program is set.

The most successful and popular healthcare reform that has already taken place started in 2004 – Health Savings Accounts. This is a special account kind of like an IRA, where anyone with a qualifying high-deductible health insurance plan can put aside pre-tax money to cover future medical expenses.

Health Savings Accounts encourage people to save for their future, and when they do need to tap into the money to pay for medical expenses they have much greater incentive to be careful how they spend that money. It is this consumerism that facilitates price among providers, which helps keeps costs down for everyone.

HSA plans have become very popular, in part because they are much less expensive than conventional copay plans. So many people are concerned that these high-deductible plans and Health Savings Accounts will not be available after healthcare reform passes.

Since there is no final bill yet at this time, we can only speculate what will happen. But it does look like yes, Health Savings Accounts and high deductible health insurance plans will probably still be available under Obamacare.

The first clue that these plans will still be available is the minimum benefit levels defined in the Baucus proposal in the Senate. The bronze benefit package would allow an out-of-pocket limit up to the HSA current law limit ($5950 for individuals and $11,900 for families in 2010).

The bill also makes some changes to how distributions from Health Savings Accounts are handled. Distributions from a Health Savings Account that are not used to cover medical expenses will be subject to a 20 percent additional tax penalty, instead of 10 percent as it now stands. Not that big of an issue if you use your Health Savings Account as it is intended.

So we can see they seem to be thinking about Health Savings Accounts. But here’s the possible catch. All qualified health insurance plans “would be required to provide primary care and first dollar coverage for physician services and prescription drugs.”

Under current law, an HSA-qualified health insurance plan cannot offer first dollar coverage, other than for certain preventive benefits. So currently a plan cannot comply with the requirements in the Senate bill, and also be HSA-qualified.

Both the House and Senate bills give authority to work out this issue to an appointed official. In the House it is the “Health Choices Commissioner.” The Senate Finance Committee (Baucus) proposal gives authority to the secretary of Health and Human Services.

So while there is no certainty at this point, we are expectant that not only will Health Savings Accounts be part of the package if healthcare reform is passed, but will become more popular than ever due to the much lower premiums, the tax benefits, and the long-term saving opportunities.

Regardless of how healthcare reform works out, remember that most of new laws would not actually go into effect until 2013. So it still makes sense to get and fund an HSA plan right now. Anyone who has a qualified HSA health insurance plan in place by December 1st can make a 2009 contribution, and cut their April 15th tax bill by up to $1900 or more.

Our government has recognized that it is very important to care for its elderly citizens. That is the reason why Medicare health insurance was created. Anyone age 65 or older is eligible for this government health care plan. For many that are eligible, Medicare can be very confusing because it has many parts.

To put it simply, Part A covers any costs for impatient hospital care. Most of those who pay Medicare taxes from paychecks receive Part A for free and automatically.

Part B includes some doctors’ services and outpatient care-but it is not free. Part B requires a minor monthly premium.

Part C deals with various health care options, like HMOs, that are approved by Medicare but are not related. Part C allows for a private insurance company, through a government contract, to reside over all of your Medicare benefits. Part C also requires additional costs that can vary widely.

Part D was created to cover prescription drugs. This plan is controlled by private companies that are Medicare-approved. It is also optional and costs more money.

The “original” Medicare plan was comprised of only Parts A and B. Later, the “advantage” plans of Parts C and D were added, and with a higher cost. Parts C and D had to be created because parts A and B did not pay for everything.

Medicare has created Medicare supplement plans to ensure that all costs can be paid and it is a type of private insurance. Most of the costs these plans cover are deductibles and co-payments that can add up very quickly for seniors on fixed incomes. The supplement plans have been termed “Medigap” because the plans were created to cover the “gaps” in costs for procedures that are not always included in Medicare. Medicare supplement insurance is standardized and regulated by the U.S. government. You can obtain a supplement plan through insurance companies and you must choose which insurance company you want by the price of their supplement plan. This is true because each plan covers the same things-no matter what insurance company you receive it through.

“Medigap” insurance is regulated by the Federal government and insurance companies must also follow state laws. Due to different laws, the plans can vary slightly state by state. To add to the complexity of Medicare, there are 12 different types of standardized Medicare supplement insurance. They are designated letters of A through L, so it can take a great amount of time to figure out which supplement plan fits your needs.

Sadly, many Americans do not receive health insurance. This dilemma is often due to high insurance rates and prolonged, or even small, illnesses that can clean out an underinsured person’s savings. This results in medical bills that may never be paid off. The key is to have a plan in these situations. Even with the outrageous healthcare costs in this country, it is vital to plan properly if you were to need medical attention. No insurance that is truly affordable will ever be able to cover all healthcare costs. So, you must evaluate your circumstances and your anticipated needs. Due to the fact that medical needs may come up suddenly, it is recommended that you obtain Medicare supplemental insurance.
